chain tensioner

I adjusted my chain tensioner on my blaster today to tighten the chain. well anyways i didn't notice it for a day or two but then i pulled a wheelie (long enough to notice) that it steered to the left. i checked the psi and they are both he same. then i looked a little closer and realized my axle was crooked (not bent) so i loosened it back up straightened it out and retightened it, and it happened again. i repeated this process 3 times and no matter how tight i tighten it the axle still goes crooked.? is there something I'm doing wrong or is something bent?
 
yup both as tight as i could, which is pretty tight considering i had to use pb blaster to get them back off
 
the bolts arent loose after it happens either. idk it rides fine it just goes crooked when in a wheelie and to me sticks out like a sore thumb but no one else notices that the axle is crooked

Use a tape measure from the back side of your foot pegs to the axle on each side before and after wheely. That will tell you if your right or wrong. Just make sure you use the same points of measurement each time.
 
Strange problem, if the carrier bolts are tight, the carrier should not move out of alignment if the adjusters are intact.

Could there be a crack, break, bend in the swinger?

Will it move side to side if you flex it, maybe a lug has broken free, or the bolt which connects it to the frame, has broken or slipped out .
