Now im confused between getting the +4mm Hot Rods crankshaft OR the +3mm Vitos crankshaft.

as far as reliability i would consider them the same once you had them trued and balanced. if you dont plan on getting it balanced i would go with the hotrods but you need your cases trenched.
well then, stroking it the extra mm of the +4 will add more performance

But when i get the liquid cooled head do i have to modify it in any way?
not if your builder uses a spacer plate for the stroker instead of cutting the head wich i assume he does.
What if he doesnt have a spacer plate?
