A good sprocket and a bad chain has been the reason for the chain to throw off.
Chains and sprockets marry for life and when replaced both chain and sprockets should be replaced as a set.
I cannot help with the removal of the bearing as I have never taken on that task.
One could try the method used in removing the spigot bearing from a crankshaft.
