Jumping out of gear is usually worn engagment dogs "gears" in the transmission , hard to shift is possibly a bent shift shaft, worn shift drum or shift forks. try checking your oil level and the condition of the oil.
Start by removing the right hand side clutch cover. That means kickstarter etc.
Remove spark plug, jack the back wheels off the ground and roll them forward, while doing so, see if you can shift into all 6 gears and down to 1st again
Remove the shift shaft
Inspect the shaft for a bend
Inspect shift star for worn/damaged teeth
Pull clutch lever and watch to see if the clutch plates are moving (engaging/disengaging)
If all is well there but you cannot find gears, you going to have to dive deeper and spilt the cases and investigate worn or bent shift forks.
