piston damage

the other day me exhaust stud helicoil came out so i had to pull the top end to get it fixed anyways and this is what i found and my cylinder has a few nice lil scores nothing major but enough to feel it when i run my finger through

this pic idk if u can tell but theres a lil chunk out of the top center like it was preigniting and burning a whole but i run a b8es plug

yea that piston looks bad, how does your cylinder walls look?.

they actually dont look bad like i can feel a lil but otherwise they look not bad u can see where it seized though like where the rings stuck...... but what u think would of caused this?
