No 5th gear.

I took it apart thinking i was going to find a chewed gear/teeth missing. But all looks good. Except for this bearing keeper. Would this alone cause it not to have 5th gear? Or was it assembled wrong on the last rebuild. The keeper is out 1/8 of a inch and the screw tabs are broke off.

Check the bearing. Does it roll freely? Did outside race spin in case?
It must be flush with the case. Almost looks cocked alittle in pic? Or just angle of pic?
When case's are heated and new bearings installed. That bearing has been known to move. It doesn't have a land that it sits on.
 
Also look at fork ends for excessive shiny area on ends. Basically see if one is bent . main shaft may have been moving some
 
It looks like shift lever number two has a little bit eaten out of it. At least I think that's the number for the single lever arm I would have to double check. is it worth replacing all these or does it look like only the one that's worn needs to be replaced? Thanks for the help.
